The denial first-steps page
A denial letter feels like a verdict. It is actually a citation: the carrier is pointing at one rule in your policy form and saying this one. Your first job is not to argue — it is to find out whether the rule they cited actually says what they claim, on your pet's actual dates. This page is deliberately plain about what we are: a first-steps guide, free, with the state complaint door behind it. (A dedicated free site, PetInsuranceDenied.com, publishes per-carrier appeal procedures in depth; the appeal basics are free all over this market, and anyone charging you mainly for an appeal letter is charging for the wrong thing. The paid question in a denial is almost always the next decision — stay, fight the record, or switch on the right date.)
Nationwide's appeal window is stated in your policy documents and the denial letter — calendar it first. If your policy was a legacy plan, keep the declarations page: which chassis you were on decides which rules apply.
Appeal windows by carrier, as compiled from the carriers' own procedures (checked Oct 20, 2026): Embrace 14 days · Pets Best 60 days · Figo 90 days · Trupanion 180 days. If your carrier isn't listed, the window is in your policy and on the letter — find it today.
Nationwide denials have a structural wrinkle the other carriers don't: the legacy Major Medical plan pays from a benefit schedule — a fixed amount per condition — not a percentage of your bill. Owners often read a schedule payment as a wrongful denial when it is the plan working as written (which is its own problem, but a different one). True pre-existing and waiting-period denials — including its non-waivable 12-month cruciate wait — turn on dates, as everywhere.
The denial reasons in this market reduce to a handful: the condition predates coverage; it fell inside a waiting period; it is the other side of a bilateral pair; it is chronic and outside any cure pathway; or the plan pays a schedule amount rather than a percentage. Each one is checkable against the form language and your vet's dates — the carrier rules page prints the rules as read from the forms.
If the carrier's answer still contradicts its own form, the escalation that costs nothing is a complaint to your state insurance department — regulators track these, and carriers answer them on the record. All 51 doors, with department names, are on the complaint doors page.
A denial is data: it tells you exactly how this carrier reads your pet's record. Whether the right response is to fight on this policy or to move on the computed date, the arithmetic is the same — run your pet's conditions and dates through the Switch-Check and see what every other carrier's form does with the same history.
Run your own pet's dates. The free Switch-Check takes your pet's conditions and their last symptom/treatment dates — and returns the per-carrier verdicts, the re-open dates, your earliest safe switch date, and the stay-or-switch math. Run the Switch-Check →
